A revision starts with what is already in the scalp: how the previous grafts were angled, how much donor area is left, and what scarring the first operation created. The most frequent complaints are a hairline set too straight or too low, grafts placed at the wrong angle, and density that ran out halfway back. Each of those is a different problem, and some need existing grafts removed or redirected rather than simply adding more.
